new patient information
Our physical therapists assess and treat movement dysfunctions and improve the way you move. In order to achieve that, we may need to decrease your pain, strengthen certain muscle groups, perform hands on techniques so that joints move more freely, cue you to move differently, or help you change some lifestyle habits. We look forward to working with you to help you achieve your goals.
At Performance Physical Therapy, you will be evaluated and treated by a physical therapist for one hour on your first visit. During that time we will be able to assess how your condition is affecting your life, why you aren't moving optimally, and develop a plan so we can work together to return you to optimal movement. This includes exercises to do at home, how to change the way you move, and how often we will see you in clinic. At Performance Physical Therapy, we work in teams of physical therapists and physical therapist assistants to ensure consistency during your care.
To prepare for this meeting, there will be some paperwork just as there always is in medicine. You will need to fill out a medical history, insurance verification and a questionnaire that will be specific to which body region is most affected. This paperwork can be emailed to you so it can be completed before you arrive, or you can arrive 30 minutes early to complete them.
Please arrive ready to move and exercise. This includes wearing appropriate, comfortable clothing so your movement won't be restricted during your assessment. Also east and drink so you are physically prepared. We are excited to work with you! Please let us know if you have any questions prior to your appointment.
